Finally, The suggested dose is 60-80mg/kg/day given in divided doses administered every 6 hours in non-obese children with normal renal function who are 3 months to under 12 years old.

The suggested dose for non-obese children aged 12 and older is 60–70 mg/kg/day, administered in divided doses every 6–8 hours.

Decreased Vancomycin dosage in older patients and those with compromised renal function.

For deemed fat Higher doses is also necessary.

The kidneys metabolize Vancomycin, and taking too much of it at once can be harmful. Blood concentrations should always serve as a reference for an additional dose.

Then the recommendations include taking into account loading doses for very unwell patients, which are, by definition, big one-time doses given to patients starting with no vancomycin in their blood.

They advise a loading dose of 20–35 mg/kg/day for non-obese people with normal renal function, with a maximum dose of 3000 mg in a single dose.

Still, Avoid loading doses for newborns and children who are not fat.

If the adult patient is obese, loading dosages of up to 3000 mg at a rate of 20–25 mg/kg based on real body weight may be considered.

The recommended dose is 20mg/kg, depending on the total body weight of pediatric patients who are obese.

Still, Vancomycin has a limited therapeutic window, making it simple to give a patient an inadequate or excessive dosage.

Obviously, administer Vancomycin IV normally every 6 to 24 hours.

So, Neonatal can receive it every eight hours.

Hence, Vancomycin has a half-life of 4 to 6 hours in healthy people, but it can last up to 7.5 days in those who don’t have functioning kidneys.
